Accident Takes Life of Trinity Man, ONALASKA, August 3, 2013 - At approximately 2:50 am on August 3, 2013, a red 2009 Dodge 1500 pickup was south-bound on FM-356. Just after passing Hansen Hardware, the driver, Jefferson Dale Forisha, 36, of Trinity, left the pavement to the left. His vehicle struck a culvert and sent it into a roll / spin. The vehicle rolled 3 times and impacted two large trees before coming to rest upside down facing west at the edge of the woods. Americare Ambulance Service was dispatched to the scene along with the Onalaska Volunteer Fire Department and Corporal Joshua Alexander with the Onalaska Police Department. Additionally, Onalaska Police Chief, Ron Gilbert and Corporal Ramey Bass with the Texas Highway Patrol and Deputies with the Polk County Sheriff's Office were dispatched including Sergeant David Ramsey, Byron Dunaway, and Brooke Saunders. Sadly, Forisha was killed during the collision. Traffic on 356 was partially blocked during investigation and cleanup. Justice of the Peace, David Johnson, performed an inquest at the scene. A first-call car from Cochran Funeral Home transported Forisha from the scene. According to investigating officer, Josh Alexander, why Forisha left the pavement remains under investigation. A rollback wrecker from George White & Sons Wrecker Service recovered Forisha's vehicle and it will be stored in Onalaska.
Jefferson Forisha's 2009 Dodge Ram rolled 3 times and struck 2 large trees before coming to rest at the edge of the woods on FM-356 just a short distance past Hansen's Hardware.
